Many organisations and government departments rely heavily on both paper and electronic forms for both internal purposes and dealing with their customers and the general public.  Poorly designed forms can be time consuming for the user to complete and result in errors.  The cost of correcting such errors can be significant and some organisations spend millions of dollars rectifying these errors.

However, effectively designed forms are easy to complete, follow a logical flow of information, and reduce the potential for error and hence the cost of rectifying errors.
